First and foremost, if you have not already, be sure to make initial contact with staff by calling 502-966-4104. The staff will take your name and log your time of arrival. Please at that time give the staff a brief statement on how your pet is doing. If your pet is critical in nature we want to try our best to expedite care.

Symptoms that need to immediately be brought to our attention are:

  1. Difficulty Breathing
  2. Lifeless Demeanor/Collapse
  3. Seizures
  4. Swollen Abdomen
  5. Abnormal Gum Color
  6. Cold Extremities or Body
  7. Large Wounds
  8. Active Bleeding

If you are uncertain about these symptoms or if your pet is critical in nature please address concerns via phone communication with staff.

If your pet is stable enough to wait please do the following:

  1. Fill out the registration form in its entirety.
  2. Once completed please submit and it will generate an email that will be sent to both you and our hospital. Staff will be checking emails regularly. Please call our front office upon completion of the form.
  3. Wait in your car. We will make contact when we are ready to take your pet in for evaluation.
    *Please have your cell phone readily available at all time - this is how we will be making contact.

* If your pet’s condition seems to be deteriorating please call us and let us know of the changes taking place. *

Otherwise bear with us as wait times have been exceptionally long. You should expect a minimal wait time of 1 hour. However, with the high volume of patients and with the new protective precautions in effect we have seen wait times as long as 4 hours. Please refrain from calling staff in regards to wait times. You may call to check on wait times or to ensure your paperwork not lost as this does happen because of wind or other outside factors but refrain from doing this UNTIL 1 hour has passed. The high number of phone calls has been putting strain on the already very busy staff. Also, do not leave the premises until your pet is discharged or otherwise advised to do so.

I agree and understand that it is the policy of this hospital to receive payment as services are rendered and that a deposit will be required upon admission to this hospital for patient treatment. I state that I am over 18 years of age, and I am the responsible owner of this pet and represent any and all other owners. I also give Jefferson Animal Hospital permission to share any and all information in my pet(s) medical record with my regular veterinarian. Additionally, I give my regular veterinarian permission to share my pet(s) medical record with Jefferson Animal Hospital. I give the staff and management of Jefferson Animal Hospitals permission to use my own or my pet(s) photo for promotional purposes as they see fit. I understand that I will not be compensated for allowing the hospital to use my pet’s name and likeness, or my own or my family’s likeness.*
